Goals for the Condell facility, in conjunction with the strategic plan, include the consolidation and growth of smaller medical practice sites into more highly visible larger facilities to enhance service to patients, provide operational efficiencies and promote growth for the organization.
The new facility consolidates current providers into a single location in close proximity to Advocate Condell Medical Center. The clinic is planned to house all clinical functions and most support functions.
Part of the strategic plan also includes the Core Module, a standardized clinic layout to be utilized as the typical floor plan for planning and design of outpatient clinics within the AMG clinic projects.
Additionally, the AMG clinic will follow current Advocate branding standards such as signage, interiors, systems and room standards.
This collaborative effort, led by HDR Boldt, involved over 100 team members from Advocate, AMG, as well as team members from HDR, Boldt and KJWW working side by side to innovate, standardize and implement these standards to be continuously improved upon for years to come. The process involved a series of workshops called Synapse Visioning and Rapid Improvement Events. The workshops focused on four platforms in order to realize the Care 360 Integrated Ambulatory Care Model: patient experience, operations, technology and sustainability. The Care 360 Integrated Ambulatory Care Model provides guidance for development of outpatient clinics throughout the Advocate system.
